The Louisiana Department of Health is seeking provider and stakeholder engagement, feedback and partnership for a Centers for Medicare and Medicaid Services (CMS) funding opportunity focused on improving maternal health.

Through the Transforming Maternal Health (TMaH) model, CMS will select up to 15 state Medicaid agencies to receive funding to focus exclusively on improving maternal health for people enrolled in Medicaid and Children’s Health Insurance Program (CHIP). The model will support the selected state agencies in the development of a whole-person approach to pregnancy, childbirth and post-partum care that addresses physical, mental health and social needs. 

LDH Proposal

LDH plans to apply to CMS to become one of the state Medicaid agencies participating in the TMaH model. If selected, the agency could receive up to $17 million over 10 years. The submission deadline is September 20, 2024. Awardee announcements are anticipated in mid-December 2024.

How to Get Involved

  • Attend a Meeting – join one of the informational meetings hosted by Louisiana Medicaid to learn more about the TMaH model and the additional services associated with the funding portion of the model. Details below.
  • Provide Feedback – share thoughts and suggestions through the online feedback form below, or by email at TBD.
  • Partner with LDH – consider joining LDH as a partner in the implementation of the TMaH model.

Meetings for Stakeholder Partners

LDH will host a series of meetings to provide information about the TMaH model and gather feedback from potential partners.

    • Who Should Attend: Obstetrician-gynecologists (OB/GYNs), midwives, physicians, fetal medicine specialists, nurses, clinical and support staff, doulas, lactation consultants, perinatal community health workers (CHWs), hospitals, OB/GYN practices, safety net providers, Federally Qualified Health Centers, rural health clinics, Tribal sites, birth centers and other sites of care.

During the webinars, LDH will provide an outline of the TMaH model requirements and an overview of our NOFO proposal. Attendees will have the opportunity to share feedback and offer suggestions for other activities they consider essential to the proposal.
